Tóm tắt

Tái phát bệnh cầu thận tiên phát sau ghép thận là một trong những nguyên nhân dẫn tới suy chức năng thận ghép. Bệnh thận IgA được cho là tái phát phổ biến sau ghép thận. Chúng tôi báo cáo trường hợp tái phát bệnh thận IgA trên thận ghép tại Khoa Thận - Tiết niệu, Bệnh viện Bạch Mai. Bệnh nhân nam 20 tuổi, sau ghép thận 3 năm, vào viện vì creatinine máu tăng và có protein niệu. Sinh thiết thận ghép cho thấy lắng đọng ưu thế IgA ở gian mạch cầu thận. Bệnh nhân được chẩn đoán tái phát bệnh thận IgA trên thận ghép và được điều trị với thay huyết tương, corticoid liều cao và duy trì với corticoid, everolimus và tacrolimus. Tiến triển thuận lợi và bệnh nhân có chức năng thận cải thiện và protein niệu giảm.

Summary

Recurrence of native glomerulonephritis after kidney transplantation is one of the causes of graft loss. IgA nephropathy reccurence is assumed to be common after renal transplantation. We have reported a recurrence of IgA nephropathy on kidney transplant at the Nephrology Department of Bach Mai Hospital. A 20-year-old man, 3 years after a kidney transplant, was hospitalized because of increased serum creatinine and proteinuria. Biopsy graft showed IgA deposition dominant in mesangial glomerular. The patient was diagnosed recurrence of IgA nephropathy on kidney transplantation and was treated with plasma exchange, high dose corticosteroids and maintenance with corticosteroids, everolimus and tacrolimus. The patients has improved renal function and decreased proteinuria.
