Tóm tắt

Mục tiêu: Đánh giá kết quả phẫu thuật của 42 trường hợp ghép thận tại Bệnh viện Trung ương Quân đội 108. Đối tượng và phương pháp: Nghiên cứu mô tả tiến cứu thực hiện trên 42 bệnh nhân suy thận mạn giai đoạn cuối, được ghép thận tại Bệnh viện Trung ương Quân đội 108, từ tháng 12/2016 đến tháng 9/2019. Kết quả: 42 bệnh nhân được ghép thận lần đầu, từ 41 người cho (3 người cho chết não); tuổi trung bình 41,6 năm; 39 bệnh nhân nam (92,9%) và 3 bệnh nhân nữ (7,1%). Kết quả phẫu thuật thành công 100%; 42 thận được ghép vào hố chậu phải; tất cả mạch máu được nối kiểu tận-bên, 100% niệu quản ghép nối với bàng quang theo kỹ thuật Lich-Gregoir cải biên có đặt JJ-stent niệu quản ghép. Tắc động mạch thận ghép trong mổ 2 bệnh nhân (4,7%); nhiễm khuẩn huyết sau mổ 1 bệnh nhân (2,3%), thải ghép cấp sau mổ 2 bệnh nhân (4,7%), hẹp khúc nối bể thận-niệu quản ghép phải mổ lại 1 bệnh nhân (2,3%). Thời gian theo dõi trung bình 14,9 tháng; đến tháng 9/2019, 40 bệnh nhân khoẻ mạnh với chức năng thận ghép bình thường, 2 bệnh nhân tử vong (4,7%). Kết luận: Nghiên cứu 42 trường hợp ghép thận lần đầu thấy kết quả tốt, tỷ lệ phẫu thuật thành công 100%, tai biến 11,9%; biến chứng 30,9%.

Summary

Objective: To evaluate the surgical result of 42 cases of kidney transplant in 108 Military Central Hospital. Subject and method: A descriptive prospective study was performed on 42 patients with end-stage renal disease. All patients were undergone kidney transplantation, from Dec 2016 to Sep 2019. Result: 42 patients with end-stage renal disease were received 42 allograft kideys from 41 donors (3 brain-dead donors). There were 39 male (92.9%) and 3 femal patients (7.1%) with mean age 41.6 years. The operation succeed in 100% with 42 allografts placed in right pelvic fossa; all vascular anastomoses were end-to-side type. We used the modified Lich-Gregoir technique with ureteral JJ-stent to reimplant the ureter into the bladder. Intraoperative graft arterial obstruction was found in 2 patients (4.7%); 1 patient with postoperative urosepsis (2.3%), 2 patients with postoperative rejection (4.7%); stricture of graft’s ureteropelvic junction was in 1 patient (2.3%) requiring re-operation. Duration of follow-up was 14.9 months up to Sep 2019; 40 patients are being healthy with normal graft’s function, 2 patients was dead (4.7%). Conclusion: The study of the 42 cases of kidney transplant showed good results; the success rate was 100%; the rate of intra-operative and post-operative complication were 11.9% and 30.9%, respectively.
